Connected Care is a program that works to enhance experience with transitions and build confidence, competence and shared understanding among children with medical complexity, their families and health care providers. Located at SickKids, Connected Care offers a range of services and partners across the continuum of care to coordinate, improve health and safety, spread standards of paediatric practice and deliver greater value across the health system.

Education for Home Care Nurses.

Specialized Paediatric Education for Community/ Home Care Providers Caring for Children with Medical Complexity

Connected Care provides education for community/ home care nurses at no charge to address the unique care needs of children with medical complexity and technology dependence. Education is delivered through competency based modules & online webinars.

Effective September 7, 2021, all individuals coming on site to any SickKids premises are required to be fully immunized (an individual is fully immunized 14 days after receiving their second dose of a two-dose vaccine series). If you are not fully immunized, Connected Care offers Virtual Education.

Connected Care for Community Hospitals

To meet the demand of unprecedented challenges to access and flow for surging volumes of paediatric patients requiring acute care admissions, Connected Care is extending its services to Community Hospitals in Ontario. Services include:

  • Connected Care on the Go: ‘rapid delivery’ of teaching mannequins and education supplies for hands-on practice of skills during co-facilitated virtual education sessions.

  • Clinical Leadership Placement: observership for community hospital interprofessional education and practice leads to gain and exchange knowledge in paediatric care through observation, practice simulations and other educational activities.

  • Resource Navigation: library of self-directed education and practice resources, access to webinar recordings and consultation with Connected Care.

 

Improving transitions from hospital to home.

Committed to improving patient and family experience, Connected Care aims to improve transition from the hospital to community/ home through providing family caregiver education prior to dischargebridging transitions between care teams/settings and partnering to deliver system level transformation.

 
 

Family Education

Connected Care provides a central and dedicated family education program at SickKids focused on timely, one-on-one training for family caregivers of children being discharged from hospital on medical technologies.

Bridged Transitions

Connected Care is dedicated to improving transitions for families and home care providers through virtual and in-person visits at points of transition, focused on building confidence and comfort within the team.
