Improving transitions from hospital to home.

Connected Care provides support to family caregivers and community health care providers who are caring for children with medical complexity and who depend on the use of medical technologies. Connected Care delivers three main services: education, virtual outreach and bridged transitions.

 
 
 

Paediatric Education

EDUCATION FOR FAMILIES

Family education provides teaching support for family caregivers who have a child in the hospital and are preparing to go home. Scheduling of classes are flexible and an interpreter can be arranged if needed. Classes are available Monday to Friday from 10 am to 6 pm and Saturday from 8:30 am to 4:30 pm. Classes available include:

  • Nasogastric tube care

  • Tube feeding

  • Oxygen saturation monitoring

  • Central venous line care

  • G/GJ tube care

  • Subcutaneous injections

  • Tracheostomy care

  • Suctioning

If you are interested in a class please speak with your health care team (e.g. nurse, doctor or nurse practitioner) to discuss if this is the right option for you and your family.

Education for Home & Community Care Providers

Community education provides workshops for home and community nurses who are caring for children with medical complexity. Workshops include:

  • Paediatric Tracheostomy

  • Pulmonary Clearance

  • Enteral Feeding Tubes & Enteral Therapy

  • Home Ventilation

  • Vascular Access Devices

If you have a home care nurse that would be interested in attending a workshop, please speak with your health care team or care coordinator in the community for more information.


Virtual Outreach

This service includes use of an online platform that provides home and community nurses access to evidence-based resources and the ability to consult with a SickKids nurse through talk/text/video 24 hours/day about practice and education related questions.

If your home care nurse enrolls to use Connected Care’s virtual outreach, they will discuss and invite you to join in the conversation when they connect about your child’s care.

 

Bridged Transitions

Connected Care will work with home care agencies and families to help plan the transition home for children with medical complexity. Services may include refresher education for families and home care nurses and pre-discharge meetings where you may meet your home care nurses, review your child’s care plan and review other care needs before going home.
