Connected Care Clinic

Connected Care offers clinic visits to families and caregivers of children with central lines, NG and G/GJ tubes that do not have home care services set up upon discharge. The aim of the clinic visit is to enhance transition experience and support safe care for children at home. Visits are virtual, and can be escalated to in-person after the Connected Care Resource Nurse completes their assessment. Clinic hours are 8am - 8pm, 7 days a week.  

Clinic visits are for routine nursing care. For immediate medical attention call 911 or go to your nearest Emergency Room

 We will require the following information about your child to book the visit:  

• Child’s name and date of birth  

• Service requested

• Receiving home care nurse (yes/no) (date/time)  

• Convenient time for Virtual Visit (e.g. time of heparin locking)  

• Preferred email and phone number to reach you
