Our Family Education program provides teaching support for family caregivers who are preparing to go home or require additional support after discharge. Scheduling of classes are flexible and an interpreter can be arranged if needed. Classes are available 7 days a week. Classes available include:

Education for Home & Community Care Providers

Our Community Education program provides workshops and webinars for home and community nurses who are caring for children with medical complexity. Topics include:

If you have a home care nurse that would be interested in attending a workshop or webinar, please speak with your health care team or care coordinator in the community for more information.

Connected Care delivers a bundle of services to families and home and community care providers in support of safe transitions from hospital to home. Services include ‘just in time’ training that is personalized as needed; partnering with families, hospital and home care providers to conduct a joint review of a child’s care plan; and virtual visits within one week of discharge to offer additional education or other needed supports.
