Connected Care Nursing Week 2023 Award Winners

Connected Care wishes to recognize every member of our Community of Practice! We are privileged to collaborate with you and thank you for your dedication to delivery of evidence-based paediatric nursing care and support for seamless transitions from hospital to home and community care.    
 
This year we are continuing with our Connected Care Nursing Week Awards to recognize the exceptional contribution and partnership of several members of our Connected Care Community of Practice. Congratulations to all!  

Connected Care Good Catch Award
This award goes to a nurse who has made a ‘Good Catch’ and acted on their nursing expertise to consult with Connected Care Live in identifying a gap and delivering safe nursing practice and prevent harm coming to a child in home and community care.

Connected Care Engagement Award – Homecare Provider
This award goes to a homecare nurse who has strong engagement with Connected Care, consulting with the team to ask questions, actively participating in their education to build capacity in delivery of evidence-based paediatric nursing in home and community care.

This award goes to a leader in paediatric homecare who has strong engagement with Connected Care, collaborating in the design and implementation of systems and structures to promote best practice and delivery of evidence-based paediatric nursing in home and community care.

This award goes to leaders in the paediatric health system who has partnered with Connected Care in program development to promote a shared standard of evidence-based paediatric nursing and seamless transitions from hospital to home and community care.

Connected Care Interprofessional Practice Award
This award goes to a healthcare provider or team who has collaborated with Connected Care in development of programs that promote evidence-based paediatric nursing and demonstrates excellence in interprofessional practice, education and leadership.
