Oxygen Therapy
Bedside emergency supplies
Adolescents would fit the adult (small-large) resuscitator adult masks.
Adult-sized BVM for patients > 20 kg; manual ventilation is to visible chest rise.
Use a PEEP valve for patients with a PEEP > 10cmH2O.
At SickKids, we use a flow-inflating bag (Mapleson circuit with 1 L bag for those > 20kg), although it is not necessary. A BVM is more than appropriate for all patients.
Oxygen Therapy Options
Nasal Cannula
Face mask
HFNC
Non-Rebreather Mask
0.5 - 6 LPM
See pathway for limits based on age.
Could be Venturi, Large volume nebulizer, MistyOx
See algorithm,
Adolescents 40-60 LPM, titrate in increments of 5-10LPM
10-15 LPM (or whatever keeps reservoir inflated)
Most commonly used
Can be used if patient prefers a face mask to nasal cannula
Use when failing standard oxygen (and further support not yet indicated)
Use in emergencies desaturations; or stay on HFNC with 100% O2
Oxygen therapy is approached similarly in adults as in pediatrics.
An adolescent will have same nasal cannula limits as an adult (5-6 LPM, use your ; approaches on when to use HFNC can be the same as adults.
