Connected Care is an interprofessional program at SickKids that improves the transition from hospital to home and community by supporting families and healthcare providers to deliver safe, high quality, evidence-based and integrated paediatric care in their local communities.

Our program provides competency-based education to support the unique care needs of children with medical complexity and/or technology dependence. Care Coordination

Transitional care coordinators collaborate with clinical teams and family caregivers when planning for transfer to another hospital or discharge home and to the community. This may include support with funding programs, home equipment and out of province resources.
